Y2018-02 Disruption of information systems on 7-8 November 2017 in the Hospital District of Helsinki and Uusimaa

The Safety Investigation Authority, Finland has decided to initiate an investigation, under Section 2 of the Safety Investigation Act (525/2011), into disruptions of the information systems of the Hospital District of Helsinki and Uusimaa that occurred on 7–8 November 2017. On 21 March 2018, the Ministry of Social Affairs and Health presented the Safety Investigation Authority, Finland, with a request for an investigation, which was taken into account when considering whether to begin an investigation in accordance with Section 17, subsection 2, paragraph 3 of the Safety Investigation Act .

The operations of the Hospital District of Helsinki and Uusimaa were seriously hindered by disruptions to information communications on 7 November 2017 and to a lesser extent on 8 November 2017. The disruptions caused particular problems for patient information systems, laboratory and imaging systems, and automated drug distribution.

Safety Investigator Timo Naskali is appointed as the Head of investigation team and Kari Ylönen (M.Phil.), Petri Pommeli (Licentiate in Technology), Alpo Vuorio (Phd. Med. Sci.) and Simo Piitulainen (Licentiate in Medicine) are appointed as team members. Chief Safety Investigator Kai Valonen will serve as the Investigator-in-Charge.
